Policy HSP-703: Outpatient Mental Health Treatment Planning (Iowa DOC)
Iowa Department of Corrections policy HSP-703, effective January 2025, requires every patient receiving outpatient mental health treatment in an Iowa prison to have an individualized Mental Health Treatment Plan completed by the assigned Mental Health Provider or Psychologist in ICON Medical. A psychologist is assigned to each patient receiving outpatient care, and a prescribing Mental Health Care Provider is assigned to all patients with serious mental illness, those housed in Acute and Sub-Acute units, and anyone taking psychiatric medication. Plans must include a DSM-5-TR diagnosis, problem-driven goals with target dates, adjustment based on measurable outcomes, referrals as clinically indicated, and discharge and reentry planning, and must be reviewed with the patient. The policy defines Serious Mental Illness, Chronic Persistent Mental Illness and Intellectual and Developmental Delay, and describes the institutional Care Review Committee that reviews cases not achieving expected health outcomes or needing transfer.
