Guidelines for Addressing Your Health Care Needs (New Jersey Office of the Corrections Ombudsperson)
This 2025 brochure from the New Jersey Office of the Corrections Ombudsperson is a step-by-step guide for incarcerated people on getting medical care and on when to escalate a complaint. It explains that during a medical emergency you alert your housing officer, who contacts UCHC staff, and that calling the Ombudsperson first delays emergency care. For non-urgent matters that can wait 48 hours you submit a JPAY Health Services Request Form MR-007, and the brochure states there are currently no co-pays for sick call; submitting an inquiry or grievance instead of the MR-007 delays nurse scheduling. It covers medication refills using MR-007 with the subject line and description to use, chronic care appointments every 90 days, medical records requests on Form MR-022 which can take up to 30 days and may carry per-page fees, requesting medication-assisted treatment by messaging OSAPAS through JPAY, work injury reporting on form MR-026, ADA accommodations via the facility ADA Liaison and Form 100, compassionate release and medical transportation. It is explicit that the Ombudsperson can evaluate health care concerns but cannot diagnose, provide records or require a particular treatment.
